A degloving leg injury, where skin is torn away from the tissue beneath in a machine or road accident, needs a plastic surgeon within the first days. Dead skin must be removed, damaged veins controlled and the wound covered with a graft or flap. Patients from Bharuch can be reviewed in Surat by Dr. Ashutosh Shah.

A leg wound from a factory or highway accident can look simple on day one and prove much larger by day four. This guide to degloving injury leg treatment covers the first 72 hours and safe transfer from Bharuch.

What is a degloving injury and why does the skin look fine at first?

A degloving injury is one where the skin and fat are sheared off the deeper layer of the leg, the way a glove is pulled off a hand. It looks fine at first because the damage is underneath: the small vessels that feed the skin from below are torn, but skin takes a day or more to show that its blood supply has gone.

  • Open degloving. The skin is torn and lifted as a flap. This is the typical picture of skin torn off leg after accident.
  • Closed degloving. The skin is unbroken but has come loose from the layer beneath, often with fluid collecting under it.
  • What happens next. Skin without a blood supply turns dusky, blisters and goes black over days. Which skin will survive is hard to judge early, so these wounds are looked at more than once.

Which injuries from industrial and road accidents need a plastic surgeon? When to call a leg injury plastic surgeon near Bharuch

Any leg injury where skin has been lifted, crushed or lost over a wide area needs a plastic surgeon early, not after the first dressing fails. Skin came off my leg in a bike accident, do I need a plastic surgeon? Yes, if any of these apply:

  • Skin torn off or peeled back as a flap, even if it has been laid back in place.
  • A leg caught in a roller, belt or conveyor, or run over by a wheel, even when the skin is unbroken.
  • Bone, tendon or a joint visible in the wound, or a fracture under the wound.
  • Heavy contamination with soil, grease or chemicals.
  • Skin that turns dark, blisters or goes numb in the days after the accident.

Go to the nearest emergency department first, without waiting for a transfer, if bleeding does not stop with firm pressure or the foot is cold, pale or numb. These signs mean the blood supply is at risk.

What happens in the first 72 hours after admission?

Three things happen: dead tissue is removed, the blood supply is checked, and cover is planned. What is the treatment for a degloving injury on the leg? It is this sequence, done early and in order.

The timings below follow the BOAST Open Fractures standard of the British Orthopaedic Association, earlier numbered BOAST 4. It was written for open fractures and is used here as the benchmark for severe soft tissue injuries of the leg. Its clock starts at the injury, not at admission.

When What is done Why it matters
First hour Intravenous antibiotics, tetanus check, foot pulses and sensation recorded, wound photographed and covered with saline gauze, leg splinted. Antibiotics ideally within 1 hour of injury. The first record is the baseline.
Immediately Surgery without waiting if the blood supply to the foot is threatened or the wound is heavily contaminated. A leg without blood flow cannot wait.
Within 12 to 24 hours First debridement in theatre: dead skin, fat and muscle removed, bleeding veins controlled. 12 hours for high-energy injuries, 24 hours for low-energy ones.
Before cover Second look where needed: skin that has died since is removed and the cover is planned. Degloved skin declares itself late.
By 72 hours Graft or flap cover, where the wound is ready for it. Definitive cover within 72 hours of injury.

The second look is clinical practice, not a BOAST timing. Debridement means surgically removing dead and contaminated tissue, covered in Debridement surgery explained. A leg vein accident, where a vein is cut along with the skin, is dealt with at the same operation.

In our practice, Dr. Ashutosh Shah plans the second look at the end of the first operation instead of waiting for the dressing to show a problem. Its timing is set by how the skin edges looked when that operation ended.

How is the leg covered once the dead skin is removed?

With a skin graft when the wound bed is healthy muscle or fascia, and with a flap when bone, tendon, a joint or metalwork is exposed. The choice is made after debridement, not before.

  • Reusing the torn skin. Skin that will not survive on its own can have its fat removed and be put back as a graft, a technique described in published plastic surgery literature.
  • Split-thickness skin graft. A thin layer of skin taken from the thigh. It takes well on a clean, healthy bed.
  • Local flap. Nearby skin and muscle moved across with its own blood supply.
  • Free flap. Tissue moved from another part of the body, with its vessels joined under a microscope. See Free flap cover for leg wounds.

A negative pressure dressing often keeps the wound clean between operations.

How should transfer from a Bharuch hospital be arranged?

By phone first, with the records sent ahead, and only once the patient is stable. Where can I get leg reconstruction near Bharuch? Patients from Bharuch are reviewed in Surat by Dr. Ashutosh Shah, a road transfer away.

Transfer checklist for Bharuch hospitals:

  • Documents. Referral note with the time and cause of injury, treatment given, operation notes, and ESI or insurance papers.
  • Wound photos. Taken when the wound was first exposed, before dressing.
  • X-rays. Films or digital copies, plus CT if done.
  • Vascular status. Foot pulses, colour and sensation, with the time recorded.
  • Antibiotics given. Name, dose and time of the first dose.
  • Tetanus status. Whether cover was given, and when.

Before the ambulance leaves:

  • Call +91 83205 00350 so the team can confirm where the patient will be received. Details are on Contact the Surat clinic.
  • Keep the leg splinted and raised, and the wound covered. Do not stitch loose skin back tightly.

Travel, stay and follow-up for families are covered in the Outstation patient guide.

Next step: degloving injury leg treatment is time-sensitive. If skin on the leg has been torn, crushed or is changing colour after an accident, ask the treating hospital to send the photographs and records today.

This page is for education and is not a substitute for examination. The timings quoted are from the BOAST Open Fractures standard, written for open fractures, and your surgeon sets the plan for your injury. No cost, length of stay or return-to-work date is given, because none can be known before the wound is seen. ESI and insurance cover depend on your scheme and referral. In an emergency, go to the nearest hospital first.

FAQs

Can torn skin be stitched back in place?

Only when its blood supply is intact, and that is hard to judge on the first day. Torn skin stitched back under tension often dies and has to be removed later. Skin that will not survive can have its fat removed and be reused as a graft.

How many surgeries does a degloving injury need?

Usually more than one. A common sequence is a first debridement, a second look to remove skin that has died since, and then a covering operation with a graft or flap. Larger or contaminated wounds can need further stages. The plan is revised each time the wound is seen.

Does ESI or company insurance cover reconstruction?

ESIC states that there is no ceiling on treatment expenditure for an insured person. ESI care is normally arranged through ESI facilities and their referrals, so cover at a private centre depends on that referral and on your employer's policy. Confirm it in writing before transfer and ask for a written estimate.

When can the worker return to standing duty?

There is no single date. It depends on whether a graft or a flap was used, whether a bone was broken, and how the leg tolerates being kept down. Expect weeks to months, with a staged return: seated work first, then short spells of standing. A fitness note follows examination.

What if a vein was also cut in the accident?

Bleeding from a cut vein is first controlled with firm pressure and elevation. In theatre, a small surface vein is usually tied off, while damage to a major vein or artery needs urgent assessment. Vein injury treatment is planned together with the wound cover.

Which reports should the local hospital send with the patient?

The referral note with the time and cause of injury, wound photographs, X-rays, the record of foot pulses and sensation, antibiotics given with their times, tetanus status and any operation notes. Families looking for a leg injury plastic surgeon near Bharuch can send these on WhatsApp before the ambulance leaves.
